Forms for Summer Day Camp
The safety of your child(ren) is our #1 concern. Please complete all necessary forms and provide them to the instructor on the first day of summer camp. Failure to provide these forms to Science Explorers will result in your child’s inability to participate in the summer camp activities. If you have more than one child please fill out a separate form for each child.
Individuals needing special accommodations for disabilities should contact Science Explorers one week prior to the camp start date.
